Nicotine Patches

You are eligible to receive a 3 month supply of nicotine patches if:

1. You enrol in the QuitPathPlus program and attend all four counselling sessions

2. You are
   a. 18 years or older,
   b. not pregnant, and
   c. do not have:
      - cardiovascular disease
      - psoriasis,
      - dermatitis (atopic or eczematous),
      - active peptic ulcers,
      - severe renal impairment,
      - accelerated hypertension,
      - hyperthyroidism,
      - pheochromocytoma,
      - insulin-dependent diabetes mellitus

If any of these conditions apply to you, you may be eligible to receive the nicotine patches subsidy, if you present a note from your family physician that indicates permission to use nicotine patches.

Eligibility for the three-month supply of nicotine patches is once a year.
